This is a wonderful place to bring your loved ones. Being in the healthcare industry myself I have had the opportunity to experience the attentiveness of each staff member at Jacob's, the place is clean, friendly, and there's always fun activities to keep the residents engaged and social. I would highly recommend doing a tour to appreciate all it has to offer.

My mother in law was at this place for rehabilitation after a fall. Her knee had been badly scrapped and they were tasked with keeping the wound clean. They didn't do that. She ended up with mrsa.
When we pushed the call button for help no CNA or RN ever showed up. I had to go out and find someone.
They were understaffed and had 1 CNA to 11 patients.
The parking lot is very small and more parking was available if you walked up a hill. I came to discover that the staff was parking in the visitor parking lot so THEY wouldn't have to climb up the hill. I was there everyday for a week and the same cars were parked in the upper lot. I caught staff walking out several times and going to their cars for breaks and just sitting in their cars in the upper lot.
The quality of care was not the best. When I asked about patient to nurse ratio and was told it was 1 to 11 I asked how was it prioritized. I was told the loudest patients got the attention. Meaning the people screaming and trying to get out of bed were taken care of first. None of that applied to my mother in law. So she got less attention.

Horrible HORRIBLE facility. I'm currently here for a month to get my IV antibiotics and I have one more week. I've constantly asked for vitamin's and probiotics that I have never received. My I get my IV antibiotics 3 times a day and every single time I have to track down a nurse to remove it when finished. I've had to ask them to wash their hands and wear gloves on multiple occasions. If I wasn't able to advocate for myself I would probably be lost in translation. It's not fair how they treat older people on their death beds, leaving them soild for hours. My roommate fell out of his bed and layed on the floor crying for help. This place should be shut down. And the social services doesn't do a damn thing but give you attitude. When I get out of here I plan on pressing charges for neglect.

A huge thank you to the staff and medical teams at Jacob Healthcare. I was sent to Jacobs after a life threatening incident landed me in the hospital for 3 weeks. I was in the ICU and was then put in a less intense hospital room for further recovery. I live in another state and wasn't able to return home via plane without rehab and therapy, so I was sent to Jacob.
I didn't know what to expect when I arrived at Jacob, I was pretty scared and overwhelmed. My family had been traveling between my home state and CA and couldn't be there when I was transferred from the hospital. I had no idea what to expect. From the moment I arrived, the staff were warm and welcoming. Jose made sure I had everything I needed and took the time to talk to me and reassure me that any concerns I had were addressed.
The facility is light and bright and reminded me of a Palm Desert hotel. I had a single room facing a courtyard which was sunny when I wanted it to be and shady when the blinds were closed.
Every morning I was greeted by a CNA who cheerfully made her rounds and always made me smile. From the occupational, physical, nutritional and numerous other therapists who participated in my care, I always felt listened to and supported. Their goal was the same as mine: get me well enough to travel home.
The care at Jacob was professional, compassionate and thorough. A potentially frightening situation was made a bit less daunting during my stay there. They have a large team of specialists, therapists, doctors, nurses and support staff and I felt safe in a supportive environment that put my wellbeing at the forefront. While I can't say I enjoyed being there, I will miss the kindness and care of my amazing care team. Please be sure to have someone available on site if your loved one needs paper signatures. If you aren't keeping track, they will sign papers for them, in which case some are life altering decisions.
Additionally, there is a common occurrence of sedation medicine in lieu of medication, which unfortunately led to the passing away of my mother in law. She was rushed to the hospital during her stay after her cardiologist determined she was sedated and not given her medication.

Call it like it is. Jacobs is known to allow patients to suffer. If they aren't getting the money or funding, the care they provide is minimal if that. Unfortunately after a state investigation, it was determined that paperwork was forged by the staff. Additionally, medicine is given that is harmful to patients that is not documented. When a patient proves medicines, with evidence, that are given that were never prescribed, given to them, it shows they are manipulating their funding by trying to speed the process of a patients passing because they aren't receiving the same level of funding previously given. Bed sores, taken off medicine to prevent a heart attack only for her to die from a heart attack within 24 hours. No attention if you use a call light. Be aware. The night staff know what they are doing because the patients guests aren't allowed at night.

Death resulted from Nurse improper care and negligence with intent to cause harm by intentional shut off of ventilator.
My mother in law passed away on November 1, 2015 due to an apparent Heart Attack, which she was on medication to prevent a heart attack while being cared for at Jacobs Healthcare center.
Within 24 hours of her death:
-coughing up blood
-swollen lymph nodes
-medications were stopped, which she was on to prevent a heart attack without cardiologist approval.
- wasn't tended to after pressing emergency button for over an hour.
Some of the other issues that happened in the time she was there, which she was only there for one month:
-Not monitoring blood pressure which resulted to immediate transfer from routine cardiologist to ER.
Had to have feet elevated for over 5 hours due to low blood pressure. Jacobs didn't monitor blood pressure according to doctors orders.
-Nurses claimed her trouble breathing was due to getting used to breathing, which in fact was pneumonia after We demanded an xray be completed on her chest.
-After she requested water to drink in the early afternoon, she never received the water until late morning the next day.
-She was being fed with 1.2 cal feeding tube, and nurse informed us that her feeding would start to be restricted including water with no explanation to us.
-Repeatedly given medications without informing patient or Renae which meds she was being given and at what appropriate times they are being given.
- Did not tend to patient after requesting assistance with over an hour going by( unless visitors were there)
- Ventilator alarm placed on silent. We came into room with Alarm on ventilator being on but was recently silenced with no nurse in the room.
- reset assistance light without asking patient what they needed.
-Got back ulcers and sores due to not turning her at appropriate times. Cleaned open sores with dry rags.
-Desk operator refused/bypassed request to speak to administrator in regards to cause of death for Mother in law.
- Forged arbitration signatures for patient. Signatures on paperwork and administrator rushed through paperwork to avoid being questioned.
Everything was documented with all issues. I am happy to write a review for Jacob Healthcare, a skilled nursing facility that I would highly recommend to anyone seeking quality care for themselves or their loved ones.
First and foremost, the staff at Jacob Healthcare is exceptional. From the moment you walk in the door, you are greeted with a warm smile and a genuine sense of caring. The nurses and aides are highly trained and skilled, and they take great pride in their work. Especially Claudia, in admissions, who is so attentive to the needs of each resident, and went above and beyond to ensure that everyone is comfortable and well-cared for.
In addition to the excellent staff, Jacob Healthcare is an impeccably clean facility. From the common areas to the individual rooms, everything is spotless and well-maintained. This attention to detail is a testament to the high standards of the staff and management.
Beyond the basics of skilled nursing care, Jacob Healthcare also offers a wide variety of activities and community events that are both fun and engaging. Whether it's bingo night or a movie screening, there is always something to look forward to. These activities help to foster a strong sense of community among the residents, which is so important for their overall well-being.
In conclusion, I would highly recommend Jacob Healthcare to anyone in need of skilled nursing care. The facility is clean and the employees are nice, but the care is not good. The medical care and intervention was almost non existent unless we pushed. My father was there for 3 1/2 weeks and they bathed him one time. If we hadn't been with him at least 16 hours a day 7 days a week, we don't think he would've survived.

I've called at least 6 times requesting to speak to someone regarding admissions for my grandfather. Each time I was told Amy, who is in charge of admissions, would call me back. Till now I still have not heard from her. One time the person I spoke with gave the phone to her and she told them to take a message. Very unprofessional. If they can not respond to a simple call, I can only imagine how it would be like to get any sort of update on a family member.

They charged people for being there when medical or Molina pays for patient care that's not right
