Unacceptable! Visit after hours and you will see how staff is not attending to patients and residents at the facility in a timely manner. According to my mother's dietary plan, a low carb meal consists of taco, rice and red beans; at least this is what was given to her. The community shower was unclean and now my mom will not go into this area. Reporting the information and trying to get the name of owner is not helpful. I contacted Paragon Health Care and was told they are not affiliated with the facility.
