Stayed here on our recent trip to Hawaii in May. The Waterfalls Inn is a lovely inn with great hospitality in a beautiful location.
I personally love Hilo, but would have to say that it is probably not for everyone. It is not very tourist-oriented so there are not a lot of big attractions. That is what I like about it. Much of the downtown area looks pretty run-down, but I never felt unsafe there, and I'm not aware that there are any serious crime issues, other than maybe car break-ins and that kind of thing.
So why do I love Hilo? For one thing I find the architecture and overall feel of the dowtown area fascinating - there are some great art deco buildings, and there is not much modern architecture, so it is like going back in time. For some reason, I find the run-down quality appealing - it is as if the climate is so rainy and humid that people just kind of give up on maintaining things, which leads to interesting sights like weeds growing on the roofs of office buildings. It has a great tropical decadent kind of vibe. There is a fantastic farmer's market - do not miss it if you are there when it is on (I think Wed. and Sat. but not 100% sure on that).
It is also close to Volcanoes national park, which is not to be missed. We also went to the top of Mauna Kea - which was something like what I imagine Mars to look like. A must if you are interested in astronomy or seeing really unusual landscapes.
If you like resorts, chain restaurants and a predictable vacation, don't go to Hilo. If you want an experience of Hawaii that is off the beaten tourist path and is unlike anywhere else, even in Hawaii - check it out.

The Waterfalls Inn B&B is a great place to stay if you happen to be on the east side of the Island. The proprietor is a great guy who helped meet every need to maximize our enjoyment of the site. Our window overlooked a creek, from which you could hear waterfalls 24 hours a day. That made for very soothing sleeping atmosphere.
Hilo itself isn't all that great, and I don't know if I would recommend visiting it. However, if you are over there, this is the place to go.
It is within a easy walk of downtown Hilo. Some parts of Hilo seem kinda sketchy though, so be aware of that. The Inn itself is in a great area and you can't hear the city at all, due to the waterfalls.
