Rated 4 stars by the Federal Medicare Administrators on the CMS Website, this place is your home away from home. Being family owned is a plus for this facility becuase every patient is treated like their own family member! Very caring, great bed side manners, and GREAT STAFF! They have so much patience and will to get the patient back to their prior level of function! I would definately recommend this place and would send my family member here. I know they can be trusted with my family member's care.

Established in 2002
Sunnyvale Health Care Center is the vision of Executive Director Herminia Chavez, who has worked for more than 30 years in long-term care facilities throughout the San Francisco Bay Area, with many of those years as a director of nursing. During that time, she found them to be too institutionalized for the kind of care they provided, and so she decided to apply her experience toward creating a center with more of a human touch. That made March 25, 2002 the beginning of not just another long-term care facility, but rather a second home where family is the key idea -- to have family involved in residents' care, and to be treated like family throughout their entire stay. So far Herminia, her family and her dedicated staff see themselves as just laying the foundation of "Family Caring for Families," and their commitment to warm, personalized care will ensure that you benefit from her vision during your stay at Sunnyvale Health Care Center.
