what a great doctor!  He's been taking care of me for a couple years now.  Easy to get to in the financial district, great staff, and very personable!  I would recommend him to anyone who is looking for a primary care physician.

He's great. humorous and professional and friendly  He may take time or not depending on your symptoms.  With me, he had to check a lot of things and he def. asked the right questions.  For someone who's been in and out of doctor's offices a lot, trust me when I say that he asks all the right questions.  Some doctors don't even ask. They just assume and prescribe you some medication and tell you to leave.
Dr. Kalshan is not like that at all.  He also gave me some samples to try first instead of prescribing them.  He said to call him should I want a prescription.  That is what I call an awesome doctor.  

I also got a next day appointment which I thought was pretty fast.  The nurse/receptionist was nice and professional.  I was just waiting for about 15 minutes in the reception area, which was not a long wait.
Downtown location and 2 blocks away from work.  that's great.  I'm thinking I might have finally found myself a family /internal medicine doctor for good.

You might wonder how a physician could go from having a five star review to a one star review.

Let me tell you:

Last year I went to Dr. Kalshan for treatment after I had an allergic reaction to food and needed a primary care physician to address my after care.  He was recommended highly to me; and, at the time I was quite impressed with his care.

At that visit, when I became a new patient, I did not have insurance.  So I paid out of pocket.

I haven't seen Dr. Kalshan since, as I've been relatively well and not in need of a general practitioner.

Flash forward to today: I am sick, sore throat, fever, bad enough that I stayed home from work.  I figure that I'm feeling crappy enough that I need to go to the doctor... and, hell now I have insurance, so it's not a concern!  Not to mention that my primary care physician, Dr. Kalshan, is listed in the Aetna directory as being covered by my policy.  Score, right?

Not so much:

I call Dr. Kalshan's office and schedule an appointment.  Then this evening I receive a call from his receptionist who says:

"Dr. Kalshan looked over your file and you were only accepted as a patient because you paid out of pocket, he won't accept your HMO.  If you'd like to pay out of pocket you can keep your appointment..."

I cut her off by telling her how FING shady that is; and, reminding her of all the people who I've referred to his office and the positive reviews I've provided.  I tell her thank you very much, I will find another primary care physician.

This is COMPLETELY shady.  I have never been denied medical attention because I had insurance.

If you don't care about that shady-ness, then by all means go to Dr. Kalshan, he provides more than adequate care... but, keep in mind that you'll have to pay out of pocket for that service.

I think it's pretty damn crappy to refuse AN EXISTING PATIENT care when she's ill - especially when that doctor who is refusing the service is listed as a primary care provider in that EXISTING PATIENT's insurance directory.

Dr. Kalshan is really knowledgable, investigates your claims and gathers the information to make his best diagnosis.  He is one of the more cautious doctors that I've met but cautious is a good thing.  He can actually detect what others cannot.  He doesn't give you the awkward doctor-patient kicks you out within the first five minutes feeling.  Great doctor, highly recommended.

Dr. Kalshan is great! I've only been a patient with him for half a year, but due to my chronic medical conditions I visit a lot of doctors, often! So, I've already visited Dr. Kalshan likely more times than most of his long term patients.

I originally picked Dr. Kalshan out of my crappy insurance's (Aetna PPO) provider directory because he was very close to my workplace in the city and because he was also listed in the SAMHSA directory. My insurance was terrible so I just paid all my office visit fees out of pocket and handled the claims myself.

Dr. Kalshan is a smart, caring doctor. He spent a fair amount of time getting to know me and my unique issues. He was understanding and never judgemental. I immediately liked him and gave him 100% of my trust. Since then, I have never been disappointed.

However, I didn't want to write a positive review only, since he already has several of those. Instead, I want to address some of the negative reviews he's been getting so people will have all the facts to make a proper decision.

First, my wife is a physician. She finished her residency 2 years ago and is starting to finally build her own private practice. This is what Dr. Kalshan did only a few years ago, so I know a lot of the behind the scenes things that are going on.

The primary complaint people seem to have is Dr. Kalshan's refusal to accept HMO insurance, and the rude behavior of his office staff. I'll address the easiest first:

Although rude office staff can make life difficult and can frustrate patients, they should keep in mind that they are seeing the doctor, not the office clerk! Do what you need to do to get your appointment scheduled, and then TELL the doctor about your experience with his staff. Its likely he doesn't know what  horrible impression his staff may be giving and he will address it! Receptionists and the like can be easily replaced quickly, but a very good doctor isn't so easy to find.

Also, in regards to rude staff, if you are having difficultly dealing with a receptionist or whatnot as in the example in one of the previous reviews with her being rude or sounding shady, you have the right to ask to speak DIRECTLY with the DOCTOR! You may have to leave a message and wait for a bit until he gets time to contact you, but you are NOT forced to deal only with his receptionist. So, if shes giving you a hard time, tell her you are done dealing with her and want to speak to the doctor!

The more complex issue, not accepting an HMO. They don't give me enough room to really explain this here but I will try. My wife, and all other docs, have discovered that HMO plans interfere with a patient's quality of care by implementing policies that directly dictate what a doctor's course of diagnosis and treatment needs to be based on studied they performed. These policies are designed to minimize patient risk while also minimizing the amount the HMO needs to pay. The HMO has a list of tests/treatments it thinks should apply to all patients, it sets how much they will pay for these tests/treatments (often far less than the doctor's or clinic's actual cost!) They are basically telling the doctor how to treat you and they don't care about individual cases, its all based on statistics. By refusing HMOs, Dr. Kalshan protects the quality of care his patients recieve.

The most important point of that, though, is that Dr. Kalshan is in NO WAY refusing medical treatment to those patients with HMO insurance. He is refusing to BILL his services to those HMOs. He will accept HMO patients but they will need to pay out of pocket, and he wants this to be very clear to his patients. You will find that MANY more  doctors are following the exact same policy (including my wife) especially in private practice since the operational costs of a private practive are already ridiculously high. Larger clinics or group practices can weather more of the financial impact they get from their HMO patients, but small practices simply can't afford it and still provide the best care. This isn't Dr. Kalshan's fault, its the HMO's, and it will only get better with insurance reforms for HMOs.

No doctor has to take any insurance, its totally voluntary, so Dr. Kalshan refusing to accept HMOs is most certainly NOT "shady!" Its simply smart, and better for the patients he does have!

I'm in love with Dr. Dan.  Wait - that's probably not appropriate. But, yes, there's no denying it. I'm in love.

What enamors me the most about him is his ability to talk to me like he's known me for years - I feel so comfortable and at ease; which is exactly how I should feel when I talk to my Dr. He's completely nonjudgmental, friendly, knowledgeable and dare I say, cool? Yes - cool; he's literally the hippest Dr. I've ever met.

I always leave feeling like I have a good grasp on what's really ailing me. I'm a bit of a hypochondriac at times and he's great about putting things in perspective for me. "NO, Julia, you're not dying. That would be your anxiety talking."

In all seriousness, Dr. Kalshan is great. I often find myself paralyzed by fear at the thought of going to a Dr. It's extremely comforting to know that I have one that I can trust and rely on.

Dr. Kalshan is great.  Very easy to talk to and I feel like i can bring up any topic.

The ONLY reason I gave him four stars is because his staff is horrendous.  They often don't pick up the phone and they always mess up or are slow on prescription refills.  In person it isn't such a problem but if you are relying on them for something you better be prepared to check up on them.

I don't like doctors. But when there's a lump in your armpit, you gotta see one.

I found Dr. Kalshan through my PPO network, and making an appointment was easy. The receptionist is courteous and prompt in replying to your messages. Dr. Kalshan as well was friendly and to the point, which is great when you're trying to squeeze in a quick visit during your lunch hour.

I would highly recommend him if you're seeking a general doctor.

I feel so so about Dr. Dan. On the one hand, he is a nice guy, and its nice that he is located downtown and you can get appointments relatively easily. On the downside, once you are able to get through to make an appointment, you will find yourself in the waiting room longer than you are with the doctor. He is quick and fast, but maybe a little too hurried and too quick to prescribe drugs.

Maybe it's just that he's not my style of doctor. I tend to prefer ones that are a little more thorough with their exams and don't rely on you to tell them exactly what you want or need. Like the others mentioned, he is no longer accepting HMOs. I have been his patient for a while now, and when I went in recently for a visit, I was chastised about the fact that I had an HMO rather than a PPO. As if that should matter in terms of the type of medical treatment he provides me, and as if its something that I can control.

I'll keep him for now out of convenience, but am looking for someone who has a little more attention to detail. Don't get me wrong, Dr. Dan is a nice guy with a friendly, easy-going bedside manner, but I just am not sure he is the right doctor for me.

Useless staff.  Based upon the yelp reviews here I thought I'd give this guy a call.  

I left a message at 9:30 a.m. with my name, phone number, insurance, asking for a call back for an appointment.  It's 11 p.m. and I haven't received a call back.

Professional?  I think not.

Dr. Kalshan rocks. Finding a good doctor is really hard - somewhere between finding a hairstylist you love (which is slightly easier) and finding your true love (which is slightly harder). I am SO happy and relieved to have found a doctor I really like and trust.

Dr. Kalshan is extremely competent, efficient and friendly. And his waiting room has all the "good" magazines - People, San Francisco, Vogue, 7X7, GQ (none of that Good Housekeeping or Health or other crap that a lot of other doctors' offices have). And his office is in the heart of the FiDi, making an appointment with the doctor very convenient.

I did not like Dr. Kalshan.  Not only does he have a very annoying sense of humor, but i think he hit my bone when giving me a shot.  

friends of mine recommended him to me, said he was 'great' and 'really funny."  I now question their judgement and sense of humor as well

Such a God send. We can joke about my bum back, high cholesterol, and blood pressure! I am a wreck for only being 25! He is serious about his practice and is a great listener. I find myself completely at ease discussing my heath issues and am very glad to have found my go to doc in the city. Its great that he gets life in the city and won't judge or berate you for throwing too many back or not going to the gym on a regular basis.

Dr. Kalshan seems to run a tight ship at his office, and I appreciate the fact that he was able to get me in and out of his office in a timely manner.  I always prefer to take care of my appointments during my lunch break if possible, and it was nice making it back to my office on time.  That being said, I should probably mention that I only saw Dr. Kalshan for a flu shot, so it's probably expected that my appointment lasted a total of 15 minutes.  However, a pleasant experience is still a pleasant experience, and Dr. Kalshan was really, really nice.  I would definitely go back to his office, and contrary to other reviewers, I thought the receptionist, Michelle, seemed very cordial.

Bonus: It was wonderful to wait in a waiting room that was so clean and well decorated.  I absolutely detest waiting rooms that are messy and gross.  Dr. Kalshan definitely gets brownie points for the Zen atmosphere! :)  Plus, I didn't even feel my shot!  That's a first! :)

Note: The co-pay is cash only (my only complaint), and he only accepts clients with PPO insurance policies.

Just got back from seeing Dr. Kalshan, and had such a good experience that I felt compelled to add my opinion.  He seems just as great as all of these positive reviews indicate.  He's very thorough and thoughtful, asks a lot of questions, answers any questions you may have, etc.   He was exceptionally knowledgeable about a range of issues, and seemed patient and interested.  Plus he's just a nice, very down-to-earth guy.

Hopefully this review doesn't seem effusive, but I gotta tell ya, I've put off going to the doctor for a LONG time.  Thanks to my wife's constant "encouragement" and the recommendation to Dr. Dan from another great doctor (Jacob Johnson, an equally superb ENT), I finally "took the plunge" - and am very happy I did.  I share in Deb I.'s relief in having found a likable and trustworthy doctor.

One more point, to those who reviewed negatively based on the office staff.  I had a great experience with them, both on the phone ahead of time, and at the office.  They were extremely friendly.  Maybe you caught them on an off-day (we all have them)?   Who knows, but whatever the case, I found them to be very pleasant.

As a Jewish hypochondriac I often find myself at the doctor. Thankfully I go to Dr. Kalshan now,  so I have less to freak out about these days.

I found him on Yelp so I felt it was my duty to write something down. I always feel listened to at Kalshans. He is very intelligent in his diagnosis and is always willing to listen to you if you feel like maybe its something else. Of course, he is usually right.

I know he accepts PPO's but I am unsure of HMO's. Check him out if your feeling sick...

Thats a weird way to end a review!

I have been seeing Dr. Kalshan for years now, and I am a huge fan.  He always finds time to meet with me, especially given that I'm sometimes only in SF for a few days before coming back to NYC.  He is honest, genuine, compassionate, a good listener - everything you would want in a doctor.  Very glad I found him and I recommend him to my friends.

I also want to give props to Dr. Dan. He rocks. He always understands where you are coming from and that you want to feel better. No B.S. However I have to agree with some one else on this post who said something about the receptionist. She doesn't quite have the best bedside manner. I had to get a doctor's note, which would have taken a few minutes and she said there was absolutely no way she could fit me in for like a week or more. I had a deadline for when I had to get the note in by as well. Boo!

The receptionist is a total bitch. When I called, the first thing she asked was, HMO or PPO? I said, HMO, and she said sorry we're not taking any. And hung up. WHAT THE CRAP??

I am just going to copy and paste someone else's review, as my experience is/has been the same.

"As great as Dr. Dan is, his office staff members are hideous.  You can never get anyone to answer the phone over there.  Getting appointments is like pulling teeth.  Getting prespcription refills is even worse...you have to call the office at least a week in advance to meet your refills at the pharmacy.  

Overall - Really bad office to work with...absolutely FANTASTIC doctor."

Dr. Kalshan is over-cautious, which when practicing medicine, is a good thing.  I've never had him ignore or dismiss anything I say as has been my experience w/other drs.  He wants what is best for his patients, not what is best for big pharm or easiest for him.

I too thought Dr. Kalshan would make an excellent PCP based on the number of outstanding positive review on here - not after my first and only appointment.

I'm a skier and with all the "great" snow we've been getting, I injured my knee.  I limped around not able to walk properly for a day or two and decided I needed to make an appointment.  I have HMO coverage (and plan to change to PPO as soon as possible after this experience), so I needed a referral.  In steps Dr. Kalshan....

I called his office about 30 minutes after they open, left a message....nothing.  Called back after lunch and she said she had called me back, confirmed my work number, and I said I didn't have any voice mails...that would be because she didn't leave one.  Ok, fine - I can't walk...when is Dr. Kalshan's next appointment.  Next week.  Really?  I can' t walk very well and I just need a referral - does he have time any sooner?  Nope - next Tuesday at 11:15.  Ok, fine.  See you then.

I arrive at 11:10 and fill out my paperwork.  Sit there.  Still sitting.  Ok, it's 11:30.  11:45.  Hmmm...maybe I should have packed a lunch.  12:00 - my turn, really??  OK!

Dr. Kalshan makes a comment about how busy he's been (I guess his way of apologizing for the wait) and is with me for about 5-10 minutes as I describe the pain and what happened.  Ok, he can't do anything for me - here's my referral.  Thanks.

Call the ortho he referred me to and they can't see me for two weeks...I can't wait that long so I call Dr. Kalshan's office back to see if he can refer me to someone else that I can see sooner.  The receptionist says she'll ask the doctor and call me back...she confirms my work and cell number.  Nothing.  

If they can't bother to call me back - I won't bother to call them back either.

Yeah, so I too went based on ratings here.  No, my socks were not blown off.  I've had more thorough doctors and a hip/friendly demeanor in a doctor is not what I'm after.  I'm guessing he knows to treat Yelpers right, but I didn't mention that I was on it...maybe I should have for more preferential treatment...

I had a wretched cold with my sinuses plugged up to the hilt.  I sat for a goodly amount of time in the waiting room.  The actual visit was maybe 15 minutes tops, charged at full first-timer fee naturally.  I felt rushed and he actually left to handle some other business and left me in the office for a fair amount of time too.  He prescribed a standard antibiotic without any tests...without even really inspecting me all that much.  In fact, he spent most of his time in his chair....talking, showing me an anatomical picture of sinuses...which I could have done without.  In the subdued light cast by what I remember to be a stained glass lamp with him sitting a fair distance away, I felt strangely like I had transported to a masseuse...without the touch.

So I didn't recover on the antibiotic.  I called the doctor's office but they didn't seem too-too interested in getting me back in any reasonable amount of time.  The staff is rather dispassionate BTW and don't seem to be in any rush.  That's about when it occurred to me that I should just bite the bullet and go elsewheres.  I had made a follow-up appt. with Kalshan but in the meantime I was able to schedule an appt. with Dr. Brian Schindler in a much shorter period of time.  When I came to Schindler he declared I was pretty badly off, took a sample of mucus, found out I had a pretty bad bacterial infection and gave me the appropriate drug to finally squelch it.  I have this unnerving feeling that if I had waited and returned to Kalshan's office for a follow-up I may not have gotten that vital test to determine what I actually had!

Dr. Kalshan is the most wonderful doctor. He was able to help me when no one else could or had given up trying. I've been seeing him for almost three years, and he always has new ideas and is up to date on all the latest product info. This is definitely the place to go if you're looking for a younger doctor that isn't afraid to try something new or natural. It's easy to get to and easy to get an appointment. The staff is great too. I can't say enough good things about this practice and recommend it to all my friends!!

I thought I had already written a review, but then I did not see it so here we go...

DR KALSHAN ROCKS.  I am so glad to have found him.  Yes, I agree that the woman who normally works in his office (who is out of maternity leave now I think) is god awful but that is not why I am there.  Dr. Kalshan listens, which is really important when you have 10 different things wrong with you, and he clearly cares.  He is also non-judgemental so you do not feel like an asshole for being "broken" which is already bad enough.  I have been to many doctors in my quest to find someone I can relate to and get good care from; additionally he calls in my refills, etc. really quickly which I like.  He is also 2 blocks from my work, super convenient downtown location with a nice office, comfortable setting, etc.  I recommend going to see him especially if you have a more complex set of issues since he has various extra training to deal with more than the average MD.  All around always feel better after my visit.

Dr. Kalshan is my PCP and I don't think it would've worked out any better.  He's one of the few doctors out there who may actually know your body better than you.

If you're a working professional and you want to see someone who gives you a clear articulate diagnoses he's the one.

Regarding his staff, it's not his fault, I think Brown and Towland just hires temps for these positions. Not sure though and I think he's trying to do something about it I see a new assistant everytime I go.

I'm tossing my old review and starting fresh.
The good news - yes, he's as good as his high ratings indicate.  I've found Dr. Kalshan to be attentive, detailed, and approachable.  The referrals he's made for me have all been to very good doctors, so he's good there too.

The negatives...
The office has had quite a bit of turnover with assistants/receptionists.  Not sure why it's that hard to keep someone, but it seems to be, so there.  Scheduling an appointment is sometimes difficult.  If you're sick, and you want to come in today or tomorrow, it's probably not going to happen.  Plan to be sick a week or more in advance, so scheduling won't become an issue, and you're set.  If that doesn't work, find another doctor, and make it easier for the rest of us to see him.

Dan is a great doctor, but we don't make the best match. . If you're like me, you might look elsewhere. I look for:

*Clear, incisive, extensive communication
*Minimalism in the use of testing procedures
*Confidence and decisiveness
*Total expertise and knowledge
*Experience

Dan is very friendly, insightful, and concerned, and he takes great care of his patients.  However, if you are looking for a very firm body of expertise and understanding that is quickly and clearly communicated, that you can quiz in order to understand what is going on and be confident that you are following the right course, he might not be the best choice.

After reading the reviews and getting tired of being rushed out of my prior doctor's office, I made an appointment with Dr. Kalshan.   I am preparing for a phenomenal trip throughout Asia and needed some malaria pills, antibiotics, etc.  Dr. Kalshan was so attentive and friendly - you can tell he really cares about his patients.  He recommended bringing some more meds like stuff for TD and made sure I had all my vaccinations.  And his assistant is hilarious - she joked around with me and put me at ease because she could tell I was nervous.  A week after my appointment I realised that I didn't ask for enough malaria pills and I just walked into the office because I was in the area.  No appointment.  Dr. Kalshan extended the scrip right then and there.  Unheard of!

Dr Kalshan is knowlegable, friendly, and trustworthy. He doesn't rush (or drag things out), he listens to his patients, and is very good at explaining a diagnosis. The receptionist is super-friendly, and the office doesn't overbook and make you sit in the waiting room for more than 5 minutes. I highly recommend him.

I've been going to Dr. Kalshan for three years and have been happy with him.  The new receptionist is another matter.  If you dare to ask for anything above an beyond an appointment or a refill be prepared for some attitude.  She will inform you how busy the office is, and make it very clear that your requests will be taken care of when it's convienient for her.  I try and think that she is having a bad day, but other reviews have mentioned similiar experiences.  I'm not sure if she's the only receptionist there, but it seems like she has too much on her plate. It's a shame because Dr. Kalshan is worth seeing, but the woman at the front desk's unprofessional attitude will lead to patients seeking a new MD.

I cannot believe I am essentially making it that much harder to get into this guy's office but his good work deserves praise.  Kalshan ROCKS and you can't do any better.  Some people below mentioned that he was hard to contact- OK, well to that I say: THE BEST ALWAYS ARE.

Believe me, if you call up and merely show up on time you will get  world class medical treatment.   I have been going to Dr. K for over a year now for a variety of ailments and I have always been given solid advice and good medicine.

No this is NOT a planted review, feel free to call me anytime with details.  Like I said, Dr. K ROCKS!!

Well, I can't really say anything else that other Yelpers haven't already covered in their reviews...all I can say is that Dr. Kalshan is highly professional, personable, and really knows his stuff. His nurse is PURE comic relief too...if you need a primary care physician, Dr. Kalshan is your best bet!

Dr. Dan is, in fact, the Man, and frankly I am a little annoyed that Yelp won't let me give Dr. Dan the ten stars he deserves. He is truly a general practitioner in the sense that he can competently handle a broad range of health problems -- few doctors can provide that kind of one-stop shopping any more. It doesn't hurt that Dr. Dan is young, well dressed and incredibly personable, but that's beside the point -- I'd like him just the same if he were a middle-aged woman who shopped at Coldwater Creek. Extra bonus stars for the incredibly convenient FiDi location, great magazine selection and Dr. Dan's hilarious assistant.

Dr Dan is the man!
I've been suffering from wicked cough attacks and scaring my co-workers away that I decided it was time to seek professional help.

I had my first appt with Dr Kalshan today and he was both pleasant and knowledgeable. He didn't want to prescribe me antibiotics unless I absolutely needed it. Apparently there is a recent study that shows antibiotics can cause breast cancer. Instead, Dr Kalshan suggested that I drink lots of fluids and get plenty of rest. The Rx was some good tasting (believe it or not) cough medicine with codeine.

Thanks to Yelp for helping me find a good doctor in the FiDi!

Oh and to Nish's point, yes, Dr Kalshan does take Blue Shield.

Dr. Dan is the man with the plan.  He really is a fantastic doctor.  Very courteous, very considerate, and takes the time to really listen to your tale of what may be wrong with you.  So many doctor's offices are like assembly lines, where you are as important to the doctor as a piece of chicken at a buffet.  Dr. Dan is much different that this...really take time with you and cares.

Now - Why only three stars?

As great as Dr. Dan is, his office staff members are hideous.  You can never get anyone to answer the phone over there.  Getting appointments is like pulling teeth.  Getting prespcription refills is even worse...you have to call the office at least a week in advance to meet your refills at the pharmacy.  

Overall - Really bad office to work with...absolutely FANTASTIC doctor.

Yes he can and he sure is Dan the man! and with your health Plan!  I was in search for a new general practioner doc. Just happen to need a doctor last week, having some fuzzy dizziness spells and naseau.   Twaz referred by 4 co-bud patients of his that have always raved about him. Even if I had already diagnosed myself online it was advised to obtain professional opinion in the event that it was not the diagnosis it could be fatal.  
+  Location Location - fidi - took me 7 walking minutes to get there : )
+  Docs overall - great personable energy - cool, calm, funny & very sincere (no white coat jitters)
+ Very knowledgeable and is on top of obtaining that knowledge

Well i can't say anything more than what everyone else is saying.  He seems to be getting ever so popular but even at that he keeps his consistancy and knows how to work with each patient, it is his gift.

and i feel blessed to have him as my new doc!

Very smart and friendly and trustworthy, he's been my personal doctor for about 3 years. I have no hesitation recommending him (I am on United Healthcare but am pretty sure he takes Blue Shield, maybe Blue Cross).
